Meeting patient needs in the home, is the goal of Alegent Health Home Care and Hospice. Compassionate staff work with patient and family members to assist them in healing and recovery in a home setting. They are dedicated to promoting, preserving, and protecting individuals’ health through quality home care, hospice and community health services.
Medication support, wound care management, personal hygiene and basic home care and nutrition services are available at different levels based on the patient’s needs. Our senior health assessments, immunizations, and health screenings all contribute to creating a healthy community. Additional services include Parents as Teachers, an education and support program for parents of children ages 0-5 and the Early Childhood Resource library.
We work with your physician to coordinate the services you need to be at home. Family Home Care staff can come to you in your home, in the hospital, or at your clinic or physician’s office to make arrangements for your care. Nursing, home care aide, and companion services are available seven days per week. Professional staff provide on-call services 24 hours per day.
Hospice Care:
Because quality of life can be more important than quantity of life, hospice care is available. With our help, the hospice patient can live their final days, with comfort and dignity, in their own home, in a nursing home and other care facilities.
Hospice cares for: People of any age with an illness causing a limited life; People who choose pain control and symptom management, instead of curative treatment; People with any terminal illness including cancer, heart disease, lung disease, AIDS, neurological diseases and Alzheimer’s.
The hospice team includes nurses, home need support, respite support and social services. Understanding the processes of end of life can be scary and overwhelming, the hospice team helps with the difficult time in life.
